Terms & Acknowledgement
1) Food
Students may bring their own snacks and drinks. The club will also provide simple nut-free snacks (pretzels, fruit snacks, fruit). For safety, no nuts or nut products are allowed.
2) Personal Belongings
Students are responsible for their own personal belongings. All craft supplies will be provided by the club. Any items brought from home should be clearly labeled with the student’s name. Club leaders are not responsible for lost or damaged personal items.
3) Medical Authorization & Emergency Care
I authorize the club leaders or DRES staff to seek emergency treatment for my child if necessary, including first aid and/or transport to a medical facility. I also authorize emergency personnel, doctors, or hospital staff to provide any treatment deemed necessary for my child’s well-being. I understand that club leaders will not administer any medication. I agree not to hold Fairfax HS, the City of Fairfax, or club leaders/staff liable in the event of an accident or injury while traveling to/from or participating in club activities.
4) Dismissal
Students must be picked up promptly at 4:45 p.m. at Door 2. Students may not walk home alone. All students must be checked out by a parent/guardian or an authorized adult, and must come to door 2 to pick up their child.
